Accessing a Survey The announcement/posting will say: "Name of BMS Course" Survey: Please click on following link to access this course's required on-line survey: Survey Completing a Survey 1.) Once you access the survey link you must complete the survey in its entirety otherwise you’ll be locked out when trying to access it later. Your attempt can be cleared by contacting Diane Blahun at x4590 or at blahun@nso1.uchc.edu. 2.) You are also required to answer all multiple choice questions in the survey. You’ll be prompted to answer any skipped questions after clicking on the submit survey button. If you are prompted to answer an already answered question, please check through the survey for other missed multiple choice questions. 3.) Do not click on the Back button in the browser or close out of the browser at anytime during the survey. 4.) Also, once submitting the survey, please do not click on the back button and resubmit - this will only duplicate your responses which we then have to delete. 5.) Do not click on the lower right submit button that is located near The Survey link. Do not enter anything in the box located below The Survey link. 6.) There may be a slight delay after you hit the Submit Survey button found at the end of the evaluation. Please be patient and once you see the "Thank You" confirmation box appear, you'll know your responses were successfully submitted. 7.) If you experience trouble successfully completing the survey from your home pc, please complete the survey on a UCHC on-site pc. Being directly connected to the UCHC server will avert any home-based pc issues. Your survey responses are completely anonymous and we hope that will encourage students to respond in a sincere and honest manner on the surveys. Applies to Basic Medical Science Courses Only: If you do not complete the survey, your exam or course grade will be held until you successfully complete it. You will be asked for your opinion about the exam, so please take the survey after your exam. You will have about a week to complete the survey so please try to complete it as soon as possible following your exam. If you have trouble entering or completing the survey, please call Diane Blahun at x4590 or e-mail her at blahun@nso1.uchc.edu. Once you have successfully submitted the survey, a screen will appear saying: "Thank you for completing this survey. Your responses have been successfully recorded." Your feedback on the Medical, Dental, and Graduate School Courses is very
valuable to us. We thank you for your attention to these instructions that
will help ensure your successful completion of the course surveys. Please
review this sheet prior to taking your surveys.
Medical, Dental and Graduate Schools’ Required On-Line Surveys Surveys
for Medical and Dental Students:
The BMS course survey
results are compiled into a summary report and sent to the Course Director and Dr. Bruce Koeppen, Dean of Academic Affairs and
Education. In addition,
preliminary summary data on the survey’s multiple choice results are
distributed to students and faculty present at the
Oral Critiques held at
end of each course’s section. Survey results are also reviewed when the
Course and Curriculum Evaluation Committee (CCEC) review a medical school
course under the chairmanship of Charles Huntington. Each course in the
medical school curriculum is reviewed by the CCEC every three years and
recommendations are delivered to the CUME. (Just
a reminder, all survey responses are completely anonymous – we can
not link a student’s name to their survey
responses – we can only tell whether or not a student has completed
the survey via Blackboard’s on-line gradebook.) 3rd
year medical students are
required to complete on-line surveys for all their MAX and Inpatient
clinical rotations along with
the Critical Care and Emergency Medicine rotations (ACE courses). This
consists in total of 15 on-line
surveys each of which you
complete at the end of that discipline’s rotation.
Results are provided regularly throughout the year to the MAX and
Inpatient Course Directors who in turn forward these to the appropriate
Section Leaders so that any issues can be corrected early on.
In these surveys, the student is asked to provide feedback on their
clinical rotation experience and on their preceptors.
4th
year medical students
taking the
Pediatrics Elective are required to complete an on-line survey at end of
their Pedi Elective rotation. Results
are then compiled into a summary report at end of the academic year. Surveys
for Dental Students:
___________________________________________________________________________________ Evaluation of Students On-Line for SCP and 3rd year MAX
and Inpatient, and 4th yr. ACE and AIE Clinical Rotations In April 2004, an off-site server based on-line
evaluation system called MyEvaluations.com was implemented for the Student
Continuity Practice and in July 2004, for the third
year MAX and Inpatient rotations. This past summer 2007, evaluations for the 4th year ACE and AIE rotations were
designed for 07-08 AY implementation. This system allows the associated community
based faculty for SCP and those Attendings teaching the 3rd and 4th year
medical students in their respective specialties, to evaluate their
students’ performances on-line.
This results in more immediate and efficient feedback to the
students on their performance, along with the students’ ability to respond
to their evaluations on-line if they so desire. For
more information about this system, click here: MyEvaluations.com System Information
